Evergreen Fountains Assisted Living wing is a two-story building attached to the main building. The apartments are luxurious and spacious with a variety of sizes, featuring studio, one-bedroom, and two-bedroom floor plans.

Each resident receives an individual assessment and a customized care plan. Evergreen Fountains works along with your personal physician to meet your medication needs. The community offers the assistance our residents need to aid them in maintaining a healthy lifestyle. The nursing Director determines the final assessment and which program would benefit the individual resident’s needs.

Residents receive assistance with Activities of Daily Living (ADLs) like bathing, grooming, toileting, dressing and more. They may enjoy daily meals, housekeeping and other campus amenities. Residents can participate in various activities and wellness programs to improve overall health and quality of life.
